Are You At Risk For Heart Disease?

The American Heart Association® recommends that everyone should be aware of their own risk factors for heart disease. Some of these factors you can't change or control; but some you can, by changing your lifestyle or taking medicine as prescribed.

Uncontrollable Risk Factors:
  • Increasing age
  • Family history and race
  • Diabetes
  • Prior smoke/second-hand smoke
Controllable Risk Factors:
  • High blood pressure
  • High blood cholesterol
  • Physical inactivity
  • Obesity or overweight
  • Cigarette or tobacco smoke

If you answer "yes" to three or more of the questions below, you should consult your primary care physician to schedule a physical exam. Your doctor may refer you to Condell's Cardiovascular Services for specific screening tests to further assess your risk for heart disease and need for treatment. If you do not have a primary care physician, call Condell's physician referral line at (847) 990-5610 to find a physician on the medical staff at Condell Medical Center.

Yes/No    Age and sex: Are you a man over 45 years old, a woman over 55 years old or a woman who has passed menopause or had your ovaries removed and are not taking estrogen?

Yes/No    Do you have a family history of heart disease? For example, did your father or brother have a heart attack before age 55? Did your mother or sister have a heart attack before 65? Did your parent, sibling, or grandparent have a stroke?

Yes/No    Do you smoke or do you live with people who smoke tobacco every day?

Yes/No    Is your total cholesterol 200 or higher? Or are you not sure what your level is?

Yes/No    Is your HDL (good) cholesterol less than 35? Or are you not sure what your level is?

Yes/No    Have you been told that your blood pressure is too high? Or are you not sure what your blood pressure level is?

Yes/No    Do you get less than a total of 30 minutes of physical activity on most days?

Yes/No    Is your body mass index in the normal range? (To find out, refer to the BMI chart link at bottom of questionnaire.)

Yes/No    Do you have diabetes?

Yes/No    Do you have a medical history of coronary heart disease? Or have you had a heart attack or a stroke?
