Condell Home Health Care provides comprehensive, intermittent skilled health care. We promote care with a multidisciplinary team approach, including case managers who are trained to coordinate care of all involved disciplines at the patient's home, including assisted or independent living facilities.What is the process for receiving Condell Home Health Care?
Upon a referral from your physician, our intake department will verify your insurance and benefits. You will then meet with a nurse to discuss a plan of treatment. Your plan may incorporate a special diet, any medical equipment you may need and your physician's expectations from the treatment. Once a plan is established, the staff will monitor your condition and review your medications. Social workers are available to provide counseling and advice for using community resources.When should a patient consider home health care?
First and foremost, you will need a referral from your physician. Consider using home health care when you need to recover from surgery or a major illness, or if you are homebound or bed-ridden.Should I wait for my physician to suggest home health care?

Medicare and most insurance companies now offer policies that include home health care benefits. Upon a referral from your physician, our intake department will verify your insurance and benefits. Condell Home Health Care accepts assignment and will handle all billing procedures for Medicare, Medicaid and many managed care and private insurance companies.Who is eligible for home health care benefits under Medicare?
All recipients of Medicare are eligible for the benefits of home health care. To receive benefits, patients must meet these four conditions:*- Your doctor prescribes that medical care must be received at home
- Your treatment involves skilled nursing care or physical therapy or speech services
- You must be homebound - which means that leaving home is difficult or infrequent
- Medicare must approve the home health agency providing your care

What are the qualifications of the Condell Home Health Care staff?
