DayhabSM... A Day Rehabilitation Program
DayhabSM offers comprehensive physical and psychosocial rehabilitation services while providing a full day of supervised activities for cognitive stimulation and socialization. DayhabSM strengthens the daily skills of walking, talking, preparing food, returning to work, driving and paying bills for those recovering from surgery, stroke, depression, trauma, chronic diseases including Low Vision diagnoses such as Macular Degeneration and Diabetic Retinopathy. The Day Center supports the goals of the patient and the entire family by providing a temporary therapeutic environment where the patient is comfortable and medically and socially supervised, allowing caregivers the ability to attend to their daily needs and obtain peace of mind. In many cases, Dayhab is less costly than in-home care and therapies. DayhabSM operates Monday through Friday from 6:30am to 6 pm.
DayhabSM therapies include:
Physical Therapy: Treatment of physical dysfunction or injury through the use of therapeutic exercise intended to restore or facilitate normal function. Also called physiotherapy.Occupational Therapy: Treatment to regain the skills of daily living and recover the cognitive and physical abilities needed to manage life’s activities.
Speech and Language Therapy: Procedures to reacquire speaking skills, recapture cognitive skills lost through stoke or other neurological injury and to address swallowing problems.
Social Work: Assessment and coordination of necessary services within the community.
Psychological Support: Assistance for individuals and family members to cope with the many feelings and fears associated with a life-changing medical condition.
In the DayhabSM program, each person receives a complete assessment upon enrollment that serves as a foundation for an individualized treatment plan designed to:
- Help patients attain maximum function and independence
- Provide for regularly-scheduled individual therapy sessions
- Include weekly multidisciplinary team conferences to ensure integration of goals in all daily routines
- Ensure regular communication with the patient’s physician for continuity of care
